Community Medical Group - Care Coordinator


Care Coordinators work alongside physicians and perform services in accordance with the physician’s orders. Care coordinators are trained to perform administrative as well as clinical duties under the direct supervising of the attending physician.

Duties and Responsibilities

Set up and maintains controls and documentation procedures related to Medicare Risk.

Adjust protocols as determined by the Quality Assurance Manager.

Analyze MRA performance data and organizing electronic medical records in accordance with company and clinical standards.

Monitor performance by gathering relevant data and maintaining MRA Database.

Code and prepare encounters to be processed by billing department.

Ensure that completed screening tests, specialist notes, operative reports are filed correctly in the medical chart prior to patient consultation with the physician.

Screen medical chart prior to visit for required HEDIS and 5 Star measures and alert physician.

Update medication list for every patient visit.

Update current problem list and past medical history for every patient visit.

Perform all duties assigned as a medical assistant: Take vitals, administer vaccines, labs, EKG, Spirometry

Receive patient calls during non-work hours

Meet deadlines on reports and other assignments

Data mining for proper documentation

Participate in large projects that might require extra hours including weekend hours.

Perform other duties as assigned



Minimum Bachelor’s Degree in Nursing or preferably Foreign Medical Graduate (MD)

Minimum 3 years of related experience in a clinic or hospital setting

Knowledge of health care and managed care administration policies

Must hold a medical assistant certification from an accredited organization.

Phlebotomy certification preferred.

Must be knowledgeable of MS Office programs.


Use relevant information and individual judgment to determine whether events or processes comply with laws, regulations, or standards.

Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines and standard accepted practices

Use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

Ability to work with individuals within and outside the organization, in professional and courteous manner.

Must develop constructive and cooperative working relationships with others.

Understand written sentences and paragraphs in work related documents.

Strong ability to use independent judgment and initiative.

Organized and able to manage competing priorities.

Resourcefulness in problem solving.

Actively look for ways to help people.

Strong written and verbal communications skills

Knowledge of medical terminology

Must be extremely detail oriented.

Strong organizational and interpersonal skills.

Knowledge of EHR

Bilingual skills preferred (English/Spanish or English/Creole)

Must develop constructive and cooperative working relationships with others.
